Well, there are a number of *large*, highly centralized,
hierarchical organizations which use it extensively :) (such as MIT
for instance). But I don't know that it "works as intended" even for
them -- usability, certificate management by users, etc. are all still
big enough issues that I'd say it would be hard to argue with you on
that point.
