I may not be required -- say in one RTT you send X packets. The
recipient acknowledges the ones it received (though some sort of bitmap
for instance). If you find, say, 3 consecutive holes in that bitmap ...
then you conclude a loss occurred and ramp-down, otherwise you ramp-up.
