i think there is an important third aspect that Enzo hints at:
c) When a centralized solution doesn't provide enough privacy.

this is the driving force i see behind future growth of friend nets
(private group networks) and dark nets. CALEA for VoIP is one example
of how privacy may become more important and large, centralized SIP
proxies unattractive.  there are better examples.
