Hrm.  From looking at the changelog.Debian.gz, I see this entry from
1.5.4+20030817-1 (17 Aug 2003):

  * Removed hcache patch.
    (Closes: #189999, #194843, #196832, #196978, #197182, #199052)
and
  * Removed libdb4.0-dev from Build-Depends (Closes: #204015)

It would appear that the functionality is not currently in the package
(At least, /usr/bin/mutt is not linked against any db libraries).
