1.  mbox format performance on insert/update is *far* slower than

2.  Mutt now has index caching (berkeleydb):

    $ ls Mail/Inbox
    cur hcache.db new tmp

Time to open my inbox (2380 messages) is about four seconds.  Updates
are likewise quick.  Most time appears to be sort.
